I haven't tried any other dialog options, such as not telling her to give it to the Qunari, so couldn't say for sure. She's returned in four different playthroughs, without me putting any thought into it, so it can't be that hard. :/

You have to do all her companion quests in that act. Including the Questioning Beliefs conversation.
You have to do her quest before Aveline's.
Tell her that you'll give her the book and agree that getting Castillian off her back is more important than the qunari.

Not true. I told her she had to give the book to the Qunari. But she still returned.

However, if you're going for friendship, tell her when she first offers the quest that any friend of hers is a friend of yours. Trying to get money from her irritates her.

But no, game wouldn't let me, and she ran off never to return. She better stay out of my way in any DLC or expansion; with both her betrayal and the deaths she has caused, my Hawke is not going to be happy to see her.
