Didn't get the option to name my dog at any point. 
That might be because I only installed the necessary DLC in the middle of my second playthrough. Perhaps you need to start a game with it installed to get to name the dog.

I name my DA dogs after Mass Effect's Krogan companions...
DA:O dog: Wrex
DA:2 dog: Grunt
