She suppose to but the game is bugged. Use the Maker's Breath potion on her after her romance to fix it.

Avaline, Merriel, Carver(Templer or Gray Warden), Bethany (Circle Robe) are the major changes
Anders, Fenrir and Isabella are minor changes (mainly just recolor).

These upgrades happen after completing companion quests? Do you also have to be a friend/rival to acquire them? I have not got them them for Fenris nor Merril, with whom I did not have friend/rival status.

Merriel I think as long as you reach friendship or rival
Fenrir I never got but people say it is from romance (I Romanced Merriel).
Isabella is the same boat as Fenrir I believe.

Merrill needs romance. I've maxed her both rival and friendship and her clothes remain the same. Oh, Isabela only needs you to sleep with her that one time. No further flirting/romancing necessary.
