Bioware thought about, but since so many people would do it, thinking their mother's death was a failure and would retry to remedy it, they figured her death was the best route.

At the beginning of act II, Varric asks you what your plans are for the future and one of the options is, something like "Protect my family". I wonder if you choose that option if you can save her?

Nope. Trust me, my Hawke tried and -- well, it seems like he's just got very ****** poor luck with family members.

I wish it was preventable, but I don't think it is. Quentin is obsessed with his wife and will always try to reconstruct her by dismembering various women.
