I took him with me on the expedition, he died on the way back, anyone else do this? Probably should have left him behind, it appears they buried him with all the equipment I had on him, lol, lost some decent accessories and a sword.
Might try keep him alive on a future playthrough.

You lose Carver at the end of Act 1, one way or another. He either dies (as happened to you), joins the Grey Wardens (if Anders is in your party in the Deep Roads), or joins the Templars (if you leave him behind). So unequip him before his final scene if you wanna keep your stuff.

If you don't take him, he gets annoyed and goes to join the templars. If you bring Anders along with you to the deep roads, he will help find Grey Wardens who will save Carver.
