on my second playthrough.
first : 3 mages (me, anders, merril) and varric
second : 2 rogues (sebastion, isabela). 2 fighters (fenris, me)
issues as I see them:
1) all the dungeons are hallways. there is no way to get lost unless you go backwards down the hall.
(perfect example : the quest to find sandal - IT IS A HALLWAY, HE's just STANDING there in the middle of it)
2)inability to control inventory of my party - so, just by nature hawke gets 4x the equipment than everyone else, including a lot of armour that is inapplicable to his/her current class and is only useful to sell.
3) liked the upgrades on armour. wish they made an actual costume change.
4) very heavily gated. for instance : dragon's blood. it's a quest reward, for a main plot quest. so if you miss it you miss an entire tier of crafting.

5) reuse of art assets - how many times do we walk down the same hallway with a couple doors closed off to make it "different" from the last time you walked down the same hallway. Sundermount is aweful for this.
6) reuse of powers -
merril's "go to ally" power is the same as back to back
ravager tree and fenris tree share the speed up on kill buff
fenris and templar both have the same pbaoe stun
merril's pbaoe damage and the ravager aura
it's like I can have 4 of characters with a *large* overlap of abilities.
