 I preordered DA2 on steam. It gave me 3 codes.
Dragon age II
Dragon age II - Entitlement Key
Dragon age II - Preorder entitlement key

I entered all 3 and do not have fadeshear sword or lion of orlais. When I tried to re-enter the pre-order entitlement key, it says its already been used. How do I fix this?

This is the Mac forum, you might want to look in the PC one. Generally writing to EA is the fix though, unless they send you an email unprompted.
