I'm about 30 hours in and only now realized that a bugged quest earlier in the game has now shorted me a companion character, Fenris. I got the quest to meet some guy in Lowtown at Night. He never, ever showed. I tried doing other things then coming back. Nothing.
I'm now at the duel, so it seems I'm long past that point now.
Fenris Quest Never Triggered for me.

You had to pick up the quest Bait and Switch in Act 1.

I did pick up the quest that started it. The NPC I was to meet never appeared and I looked online for a specific location and asked on the forums about it.
