I think it is the highest friendship not currently in your party.

I've heard if the sibling had survived, its them.

In order of priority:

1) Your love interest if they're not in the party when you speak with the quest-giver
2) Your sibling if they're still alive
3) The companion with the highest friendship rating if your sibling is dead AND your LI is in your party
