Was he always named "Dog" while Hawke was in Lothering?
so why wasn't the dog named before you got to Gamlen's house?

I lol'd at this.
Varric didn't remember the dog's name until later in the story. It's like how you sometimes have to think for a bit before you remember someone's name. Then it comes to you and you're like "I'll never forget it now!"

Because they wanted to give you one actual choice in the entire game that they wouldn't screw with.

The dog was kind of found (stray) so it would have no name it like when you buy a pet you give it a name.

In DA2 it can all be chalked up to Varric's story telling: "No ****, there I was--oh, and there was a dog too!"
