I was having major problems with the "skip to tactic" tactic.
I did some testing and found that skip to tactic is one number off. So if I tell it to "skip to tactic 11" it actually goes to tactic 10.
FYI for everyone, and hopefully the devs will bug fix.
How I confirmed it (and you can test yourself)
1. Enemy: nearest visible --> Skip to tactic 10
2. Self: Any --> Activate ability: parry
...
9. Enemy: nearest visible --> use item: injury kit
10. Enemy: nearest visible --> use ability: backstab
11. Enemy: nearest visible --> use item: poison
so what should happen is as soon as combat starts is the AI SHOULD try and backstab the nearest enemy then poison their weapon but what you'll find is that they'll try and use an injury kit. At no point should they activate parry. (actually they probably will out of combat, so manually turn the mode off in combat and it shouldn't get turned back on.)
ANYWAY, long story short: skip to tactics is broken but once you know how you can use it quite well.

Hmm, maybe that's why this was doing Chain Lightning even when enemies were not staggered?
Enemy: Staggered ---> Skip to tactic 10
Self: any ---> Skip to tactic 11
10. Enemy: Clustered with at least two allies ---> Use Chain Lightning
11. Enemy: Lowest Health ---> Attack
The use condition in next line tactics are completely broken as well.
Enemy: Staggered ---> Skip to tactic 10

I've had rather good success with condition in next line tactic actually. But I've noticed its a bit fickle with the order in which you use it. For instance 2 things I have Merrill setup to do is use a Chain Lightning on normal or higher targets if there staggered, and Crushing Prison on elites or higher, if there staggered.
When I use them in this very specific order they work:
Enemy, Rank: Normal or Higher: Use condition in next tactic
Enemy, State: Staggered: Use Chain Lightning
When I swapped it, it didn't for some reason. I know it works because I do the same thing for Crushing Prison (setup before chain lightning) to target ranks elite or higher. And that only triggers on the elites or bosses, low HP mobs she never uses anything on and normals or if crushing is on cooldown (due to crushing a LT or Assassin) she'll use chain lightning.
Basically I think, for use next condition, it matters in what order you do things. Checking for state, then rank, doesn't work but rank, then state does. Only way I use that honestly so haven't tested other combination. And I stopped using 'skip to' in DAO a long time ago. I found it easier to just use an 'attack' to cancel out sections I only wanted to happen at specific moments and with the use in next tactic thing I've removed all need for it.

Actually I just remembered one time I set Bethany up to use a next tactic line for upgraded Glyph of Paralyze. Worked great too, looked like this.
Target of Party Member: Aveline --> Use condition in next tactic
Enemies Clustered at least 4 --> Activate Glyph of Paralyzes
Worked out good, she'd use it right infront of avaline if there was a bunch on her, only ever had her use the glyph at any other moment. And it was because there was a group heading for my Rogue and Aveline was rushing over to help, Bethany used it on the group about to kill my rogue cause Aveline had 1 targetted basically. So that one worked too.
Pretty sure just the under effect part that doesn't work right, has to be a more direct targetting thing first. I'd wager you wouldn't want to use clustered first either as thats a bit more generalized. No idea why the state stuff doesn't work as first condition though, so definitely something a bit busted.
