Well, the bug itself is that a quest triggered a scene where Merrill and Hawke were talking about the Keeper's death, but the reason its a bug, is that I hadn't even started the quest yet where she sacrifices herself to save Merrill, creating spoilers inside the game itself. I'll try to explain all the things I've done involving merrill
First of all, I've romanced Merrill as much as possible throughout this playthrough, but by this point had not completed the romance of course.
I've done all of Merrill's quests in her favor. I supported her completely in the quest where you meet her, and helped her obtain the carving knife (Can't find the actual name for it on the wiki). I had just entered Act 3 and talked to her in her home for the first time. If there are any questions, please let me know, luckily I already knew what happened because this is my second playthrough, but I would've been like WHAT!? GAHHH SPOILERS if it were my first time.
Edit: Forgot to add, right after the quest came up where she was mad at herself for the keeper's death, I talked to her again and got the cutscene to start the quest for her to go see the keeper about help with fixing the mirror
