Some of my skills does increased damage against disoriented targets.  But i can't find any skills that cause "disorient".  Can anyone clarify?

rogues have some Talents that cause it. You need to check the upgrades for that.

It's one of the three class-specific status effects that also combine to create cross-class combos (just hits that do a lot of damage). Check the Codex's "Arts of War" section for a description, it's one of the only tutorial codexes that will appear regardless of whether your tutorial box in the options menu is checked.

If Disoriented doesn't appear in the cross-class combo codex, it's because you don't have anyone that can cause/has caused it. It'll show up eventually. I can't recall exactly what Disoriented does, though.

Some of the class combos are ridiculously effective. Basically you need to begin by thinking about which of your abilities you use on bosses, then look at the improved talents for that ability and find which status it exploits (from one of the other two classes). then you set your companion of that class to spam that ability.

I remember seeing as high as 900% damage on some class combos.

Winter's grasp has a relatively low % chance to apply the freeze effect to a boss, but if you are able to do it, then archer's lance will hit for unbelievable damage (may not be real example, don't have in front of me).
