She's just standing there in act 3, exclamation mark over her head, but only does the usual basic replies when I click on her. Any idea how to fix this?

It's a common bug to my knowledge. Only fix I've heard of is in the PC, where you basically skip a bunch of dialogue via console to get to the one she wants to talk about.

Blarg. I'd hate to skip dialogue, but if that's what it takes to progress... any idea where I might find such?

From the wiki page on DA2's console:
runscript zz_isa_debug
You should find yourself in a menu. Select Plot.
Select No rest for the Wicked. Act 3.
Select Jump to Plot Point.
Select First Velasco Encounter.
