I keep finding some abilities keep turning themselves off.
Berserk turns off whenever I open a menu or a frame, such as my inventory or if I press ESC.
Cleansing Aura turns itself off but I don't know when.
Any fix for these? Thanks!

I haven't tested berserk throughly, but have figured out cleansing aura.
Cleansing aura seems to ignore its upkeep value to keep it on. Since cleansing aura will consume mana everytime it heals something it can bring your mana down to 0 and deactivates at that point. I am not certain why it ignores the fact that it has an upkeep value. However, this condition only seems to take place in battle. As I've ran my mana down outside of combat and did not have the same effect.

Berserk is an uncontrolled rage... making use of the abilities provided by the menu requires control thereby ending the rage

Lol. Berserk will last you throughout the duration of a battle. But once your characters sheath their weapons, it turns off.
As for cleansing aura, yeah, busted. Not a very grand aura. I never use it because it consumes mana, and I'm better off with my actual healing spells. : /
Though yarr. Berserk works as it should, not sure about the menu business, but once a fight has 'ended' it turns off. And if you activate it before combat, you only have so long to get into a fray before it deactivates. But so long as you are fighting 'weapon drawn and enemies around' it stays on. Hope that helps.

From some quick testing I've noticed the following observations for Berserk:
Berserk will end if you manage to reach 0 stamina regardless of it having an upkeep. I think this is a real bug as I don't see the point of an upkeep if it isn't doing so.
Berserk will end at the end of every battle. Doesn't matter if you fight one mob and the combat status clears or if you fight 50 and then the combat status clears. I find it a bit annoying when you kill one mob that is a front runner for a larger group and you have to wait for the cooldown on berserk, because it toggled off after killing the first mob.
I can see the sense in it thematically, but it really does put a bit of a burr in the power.

Berserk will get turned off if you save aswell, which is annoying because it turns off then saves so I have to always wait for a CD when I'm about to enter a place where I know a large amount of people are.. So I turn berserk on and save, then berserk is off so I have to wait another 30 seconds..

It's not just beserk that has problems. The haste aura sometimes affects only one or two people in the group, requiring it to be de/re-activated. Some other sustained abilities that don't seem to run well off scripts are rock armor, threaten, telekinetic weapons and arcane shield. These are the ones I notice the most.
The deactivation of any sustained ability outside of it's requirements (mana/stamina/health etc.) is very annoying. I hope it is getting fixed.
Slightly OT, there is a similar problem with the warriors taunt. I set Alistairs script to use taunt whenever Wynne is attacked, however when it's time he just stands there indefinately with the "taunt" spell as his current action but never casts it and wont do anything till you manually intervene. The only fix is to pause and manually click it or another action. It's a shame you can't utilize it in a script at the moment. It's a problem with taunt across any player controlled character that can have it in a script. Happens with shales racial taunt ability too.

@ Amarll Berserk is only intended to work in combat, as you can't save in combat of course it resets.
@ harrier I believe Haste has an area of effect and sometimes party members are outside of it. Also, I find that my characters taunt even when I'd prefer they avoid it. Taking control of the character temporarily disables the script AI, and resets it to the top of the list again

haste doe not have an aoe that i have noticed. I have gone solo with my rogue to scout ahead and not had it deactivate on me. It happens when you load into a new area, it just fails to reinitialize on some members. I think this transitioning is also affecting other abilites such are arcane shield. Wynne has several that are set to the 'any' script condition and only 3 out of the 5 I have set are being executed when they have been deactivated by some event in the game.

Bioware has said that Berserk is working as intended in another thread (as is Rally). Berserk also has a hidden timer that will deactivate it when not in combat

I frequently use Rally and Berserk. The only weird thing I have seen is that Rally seems to increase attack damage even without the talent that's meant to modify it to do so.

Link that thread please. I find it difficult to believe, since after doing some testing of my own there are only two possibilities: Either the ability is bugged, or working as intended--assuming the intent with berserk was to create a steaming pile of cow dung.
Those who have not tested this themselves, please stop posting on these threads.
Berserk does NOT drain stamina.
Berserk does NOT have a hidden timer on it (you can activate it and leave it on indefinitely)
You do NOT have to be in combat or about to enter combat to use it.
Given that the three preceding statements are true, the ability should not shut itself off for ridiculous reasons. But that is exactly what it does.
Saving will shut it off.
Opening your area map/inventory/skills or talents list/tactics screen will shut it off.
Looting anything will shut it off.
Ending combat will shut it off (this one I can actually understand)
Etc.
If this is how BioWare intended berserk to (not) work, they seriously need to reevaluate their stance on this ability. Please people, stop defending the current state of this bugged ability. You are only making it less likely to see it fixed anytime soon.
