For me.
1: Arishok. It's a long battle and you need to plan and move alot.
2: Ser Vernal: Ugh so many mobs... This battle sucks for mages and archers, the cooldowns make this battle even more annoying.
3: Xebenkeck: Oh gow, this was just plain fricken annoying and long.
4: Brekker: It's not him that is the issue, it's his men and they can kill your whole party if your not careful, not fun.
5: Rock Wraith: Powerful attacks, lengthy, annoying creatures? La sigh.

1. Lady whats-her-name. In Sebastian Act II quest. Don't know why, but this whole quest is pretty hard for me.
2. Arishok. With chars that can't take much punishment this can be very hard
3. The pride demon during the end, in the docks.
4. High Dragon. Or rather her dragon hob.
5. Rock Wrath. Not to bad, once you have figured it out, put you really have to watch out.

I never had problems with mages on nightmare, it was those damn assassins. Any fight with 2 or more assassins is irritating.
Xebenkeck - Too many hostiles, all of which are tough.
Nexus Golem - Alot of hostiles
Ancient Rock Wraith - Partially hard to deal damage as a 2H Warrior, partially the buggy controls and constantly moving teammates
Brekker - Himself an assassin, and he has 5 other assassins with him. Only way to do it is drag them away, kill a few and come back.
Second Scroll (sundermount) - 2 Revenants. Was impossible with my casters being instantly killed and aggro impossible to maintain. Ended up dragging aggro away.

I wouldn't put Arishok in the top 5. There's so little that isn't in the player's control, it's much less stressful than ARW, Xeb, Brekker, or any number of fights with multiple threatening elites.
It's tedious, sure, but 10 Restoration Potions, 10 Combustion Grenades, and 3 Maker's Sigh should let anyone beat him without breaking a sweat.

On my second run though as a mage, on a side quest to kill a mercenary band, I was completely wiped out by an elven mage named "Leech", as in everyone died within a minute! I don't know what he did, but everyone turned black and their health just disappeared. I ganged up and killed him immediately the second time but I was shocked that this was the worst defeat I had experienced even more than the boss fights!

He use an blood magic AE on an party member. Its an red swirl on the ground. Move your party away as soon as you see this. But he doesnt cast anything if you stick 2+ ranged attackers on him as soon as you open the door. Archer assasinate will kill him too without any problem.
