I spent about 60 minutes on 2 attempts of trying to kill this guy on Hard.
me (SnS tank) + Aveline + Varric + Anders
On my second attempt I think I got him down to 25% of his health but after all my potions ran out (I started with 6 + 6) I got taken out pretty fast...
Since I can only play games for about 2 hours max per day, I decided I didn't want to possibly waste the next 3 days trying to come up with the right "munchkin" strategy to beat him so I just lowered difficulty to Casual and watched my party beat him without breaking a sweat
I did recently find a thread on this forum filled with various Nightmare/Hard tactics so you might want to look for that.

this guy is like the hardest battle in the game as far as im concerned, and he's not even that fun.

Uh, there are 4 pillars (columns) in the cavern area. When he goes into nuclear mode the pillars block some of it. If the ground is red, it's "radioactive" and you'll take damage,. but the area behind the pillar isn't red colored, so you'll be safe there.
If you're dying on Casual then probably you aren't moving your party behind one of the blocking pillars...

Even on normal you don't need to hide behind the pillars. How do people die on casual?

On my first encounter, i had my 2-h warrior, varric, anders, and isabella. Bringing Isabella was a mistake, she went down in 2-3 hits. You need only 1 melee to tank him. Bring 3 other ranged attackers to hit him from a safe distance. Would also recommend leaving on hold and turning off tactics or the AI will act dumb when he uses his gravity pull and lightning storm AoEs.

Also, keep your party from bunching up. I won this fight when I kept my people in different parts of the room, so that when he rolled into a ball and charged, he would only hit one of us at a time. This worked wonders for me.

I can't defeat him as I got fenris as tank with no spirit healer and most partymembers under 50% health due to injuries :/. most other bosses were defeatable with injured party members but not this one.

He's definitely a tough one. I was actually really nervous when I faced him on my 2h warrior because I was very close to getting Unstoppable (no deaths during an Act). Took like 9 tries, but I managed to take him out on Nightmare with no deaths eventually. 
Some tips:
* This fight is a lot easier with 3 ranged DPS allies. My most recent party was 2h warrior Hawke, Varric, Merrill, Anders. When I did it on my DW rogue, I had to try to predict his AoE abilities and back out before they went off, which made it considerably more micromanagement intensive. I did this on my warrior as well to mitigate some damage, but having to try to micro Aveline (who tanked for my rogue) as well as Hawke was taxing.
* During the add phases, have your ranged allies stand behind a column to limit the number of ranged adds that can hit them. Focus fire the adds that are in line-of-sight of them first, and then move on. This decreases the damage they take.
* After the first time he disperses into a bunch of Profanes and you kill them he starts to do the ball rolling move. Park your 3 ranged allies right beside a column as far from where you're fighting the boss as you can (usually diagonally away) and order them to Hold Position. If he starts rolling towards them, immediately pause the game and make them run BEHIND the column they're standing beside. If done correctly, the Ancient Rock Wraith will head straight towards them... and run into the column, since it is now between it and your party. This prevents them from taking any damage from it or the following AoE move he usually uses if near your party afterward.
If you already have a good party, this fight really boils down to good positioning and movement, even when he's not doing his AoE Red Light of Doom move.

This fight seems hard but it is not that hard. Think strategy!
This boss has a lot of frontal AoE abilities. And he does a lot of damage. So it would be unwise, or at least harder, to try using SnS and tank him toe to toe. His weakness is also his frontal AoE and he needs time to recharge at certain % of HP.
Strategy:
- Spread your characters in 4 corners if possible. Each corner has a rock pole/column. You can hide behind the column when he charges with his barrel roll attack, or with anything, really. But don't hug the pole, his AoE ability still hits you and send you flying far away losing a huge chunk of HP.
- When he is targeting one person, have 3 others do damage on him, but don't go all out, just sustain damage. You have to force him to certain % of HP that he has to rest and recharge. Otherwise he will go on forever and you will forever be on the run like this.
- When he needs rests and recharges, he just slums on the ground and call smaller rock wraiths to defend him. This is where you let out all your abilities. Put a Hex of Torment on him if you have, unleash everything on him. Either CC or quickly take out the rock wraiths. Even staff dps can take them out relatively fast. But I take out a few, CC the rest so I can kill them later.
Scary abilities:
- Pull of the Abyss: he will suck all of you standing in range in to him. The ability itself does not do much damage, but you should be trying to run away. Staying close to him is bad.
- Energy storm: (or something like that). When you see him curls up in the air, it is time to just run and hide behind the poles and do nothing. It will pass.
- Barrel roll: Well, just avoid it as you avoid the troll's charge. It's obvious, and run to the side, or behind pole (but stay away from the pole).
- Teleport: He will teleport around to catch some of your other characters from far. Nothing big, just run.
Pause a lot, this is not a first person shooter, so you don't need to test your reaction skill on issuing fast commands. The only reaction you need is to press Pause. Pause, watch the battle, figure out what the boss is about to do, issue commands, and unPause. On nightmare mode (or maybe hard mode too), your characters' command queue often reset upon being hit, which is a huge annoyance when you deal with being pulled in. The effect happens every second (as you can see you lose 1-2 HP) and you would have to Pause, click, Pause, click to force your people to run away. Else they just stand there. If possible, group all 4 and spam click on some distance.
Put every party on Hold at all time except when he is resting and charging up. The last thing you want is to have your melee to be heroes and face head to head against the boss. Even archers and casters would try to shoot the boss, and this is very bad when he does energy storm.
If you can do everything perfectly, you don't even need a healer. But of course, it's hard to be 100% perfect for such a long fight. So it's best to have potions and maybe Anders around with Regroup (raise) ability. As long as Anders lives, he can raise everyone up. And if the spell is on cool down, stall the boss with the above strategy.
For those who keep on losing even on Casual ... you are reminded this is not a button smasher game. There are tons of ways to go about a battle. On casual or normal, I can even win with 4 badly configured melees (yourself, Isabella, Fenris, Carver). Not to put anyone down, but I just want you to know you are looking at this the wrong way.

I found this to be the battle that required the least amount of thought.
He was barely able to deal any damage to my SnB tank, so he just focused on my Hawke while Bethany, Varric, and Anders wailed on him for half an hour.
Bethany and Anders were both set up as healers, but that was totally unneeded.

I have Varrick, Anders (healer), Aveline and Myself as a DW rogue in my party, but I keep dying before he even reaches 50% health.
My rogue, Varrick and Anders all have very low hp so they die in a few hits.
If Anders dies, I am done for. If Varrick and my rogue dies, there is no way to kill him because those are the dps'ers in my team.
Playing on hard by the way.

this guy is easy on causal at least just have plenty of health and injury potions and have two range and melle party members and use the pillarsB)

I think another tip to try is that when he summons the other rock wraiths, have your ranged attackers take them out one by one immediately. I hit pause, target a wraith and un pause. Soon as it's dead, pause, target the next one, etc.

This is not Nightmare ('tis Hard) but may help for those completely clueless as to how this fight works.

I finally managed to beat him on Nightmare without Anders.
I did it with mage hawke as primal/force/creation, Merrill, Varric and Fenris.
I made a mistake quite early which killed merrill but after 47 minutes of fighting the other 3 defeated it.

I beat this guy on nightmare with Hawke as a warrior with a 2H weapon, Varric, Merril and Aveline. The trick is to a) keep Merril alive with 'elemental weapons' sustained (combined with a spirit staff), and
have both warriors decked out with 'elemental aegis'. You'll need a boatload of potions and even more patience, but if you continue to charge in with BOTH warriors, preferable the dog as well, you will eventually wear the BIG BAD *beep* down. Keeping Varric and Merril alive is absolutely vital as even with 'elemental weapons', you won't deal much damage. You can afford to lose one warrior, but it makes it that much harder. You should try to avoid the most obvious attacks, but keeping both warriors close up and personal actually makes you take less damage than when trying to avoid every attack. Keep the pressure on this ARW and you will make your life a lot easier. Varric hugely benefits from abilities which make him fire faster. The triplet fire thing is also very useful as it allows you to take out small things in (almost) one go.
