So, I encountered this problem twice (played it three times, beat it on my last attempt) in the battle with Orsino, which is the second to last boss fight in the game.  There are open edges in that boss room, where you can run up to it and look down, but cannot pass over it due to the environment block on that geometry.  But, it is an open space, not blocked off by a fence.  So, during the boss fight, I end up by those open spots, with my group fighting along side.  Now, the first time this happened to me, it was not even during the boss fight, but during the fight prior to the encounter with Orsino, with the templars.  Fighting with a templar hunter, the rogue class templar, he would stealth, and then disappear from sight.  I first assumed he had just run to another party member, but to my agitation, after all other enemies had been killed, I noticed that I could not leave combat, and that upon further inspection, realized he had fallen through the boundary block, through this odd open space, and into the bottom of the map, and was just chilling out.  I could hurt him with some AoE spells with Merril, but it wasn't powerful enough, and I didn't want to take an hour to kill him, so I had to restart the encounter.  After my first try at fighting Orsino and losing miserably choosing 3 other party members and fighting alongside the members I left out, I decided to leave out all party members and just control myself, but have everyone fighting and not being able to die.  This may be an unforseen choice by Bioware, but it was one that allowed me to win the fight without much hassle, albeit having it take a damn long time.  But during this fight, again a rogue fell to her demise- Isabella.  Which was quite annoying to me because she does a lot of damage, and it could have gone faster if she didn't glitch off the game map.  This isn't a huge problem, but it is very annoying, and one that I think could probably be easily fixed in the first game patch.  Another thing that should be addressed is the party choice for that entire encounter; it was great to just play my warrior, and have everyone else have unlimited health, but it also meant that after the fight I was forced to play on easy, to survive the enemy encounters after the fight, seeing as I couldn't gather my party a second time.  I could have restarted the fight with a normal party, but I didn't want to.  So either players need to choose three party mates, or they are allowed to regroup after the fight.  I realize this isn't much of a technical support question, as it is bug reporting.  I didn't see a forum for bugs, so I am posting here, which seems to be the closest thing.  I hope Bioware takes notice of this.  Thank you.
