Even though I fixed the bug using a modding tool, thought I should let you know that last night...I reloaded a save before my Hawke and Isabella were friends. And then after my Hawke befriended and slept with her, I checked my abilities screen and I had the Thumbs-up ability on there! Which is kinda ridiculous 'cuz I never even used her in my party after we were friends.
Bingo, a gentleman sent me a message with almost the same senario. Were you in Hawkes house when you checked the abilites screen? I think the game automatically puts here in you're party after you romance here at hawkes estate. If this is the case, not saving/reloading while at hawkes estate after the romance should avoid the trigger.
When I get the chance I could test this but if you want to, you could save me the trouble and see if my theory is correct since you have a save after Act 1 but prior to the start of Isabella's quest (ongoing search) leading to friendship/romance.

Turns out this was as simple as the solution Hague provided:BloodyRaw wrote...

I'm still glitched after receiving her friendship (and the thumbs up), despite not putting her in the party once since I received it. There's something else to the bug, and I haven't been able to figure it out after trying scenarios for 2 hours or so. Oh well; time to kick Varric to the curb for a while, I guess, so I can properly save my game.
I don't know how this can occur to be honest. If you are in a mission/quest and recieve her friendship bonus during it, and save/reload you will encounter the bug. You have been trying different scenarios for 2 hours (isnt it a freaking waste of time), but before trying the scenarios was isabella befriended already?
If you have time please post the steps that you did here, and try not to leave anything out. We will resolve the problem.
Also, you'll get the bug from the "Beliefs" missions. The game adds and
then removes those characters from your party at the beginning and end
of those quests. To avoid this, add the characters (Sebastian or
Isabela) to your party, do the mission and then remove them before
saving. If the game forces them out of your party it screws it up.
Having them in your current party prevents it from adding them.
Everything's good now, and she's staying on the bench.

Yes, I was at the house. But, it turns out that....if you leave your home right after the romance scene, the Thumbs-up ability goes away. I actually tested this with the modding tool.
I imported the save into the modding application to check the attack speed modifier right after the romance scene (while I was in the estate)....and it had a +5 increase (which is expected). Then I left the house, and saved again just outside the house during day (had no one else in my party), imported that save into the application and the speed modifier was back to 0. So I'm guessing you can avoid the bug after exiting the house right after the romance scene.

Even reloaded twice against that big bad guy (without spoiling anything!) where she had run off.
Altho i do get the thumbs up icon showing if shes forced out of the party (hawks house aswell on load), and without taking any chances i usually just fix that by going to the hanged man, reinserting her then removing her myself.

You have the bug but if you never take her out you're party you have nothing to worry about.

So I reloaded an anterior save, made a safe party inside Merrill's house without Isabella saved, reloaded fine and then headed to the trip.
But still, once I was in the Deep Roads and reloaded in the Deep Roads (Without Isabella in my party) thumbs up was there.
I used the the savegame editor and once in Act2 was able to clear up thumbs up.
After that everythings worked fine but I suspect I'll get the same crap once I get toward Act3.

No problem gentlemen, thanks for the "thumbs up" lol. More info
The bug will START Occuring by having isabella in your party and then saving in a mission, hawks resdience, Kirkwall at night or outside the city (sundermount wounded coast) and then "RELOADING" that specific save then you will get the thumbs up bug every time you load "any" save with her "not" in the party until the end of time.
Thank You for the info. So if the bug generated but we keep Isabela in our party we shouldn't experience troubles... Since I tend to have her constantly in my party, especially due to dialogue interactions and plot reasons, this solution can be very appreciated.
I should only avoid to save and load inside Hawke's residence but I can still save before entering it.

The end of Act1 and trip to the Deep Roads triggered the bug for me, while I was free of it. Since i had Isabella in my party when you get the chance to change your party before leaving and because I didn't pick her for the trip, I thought this swap was causing the glitch.
So I reloaded an anterior save, made a safe party inside Merrill's house without Isabella saved, reloaded fine and then headed to the trip.

Wow Crazy. I never reloaded in the deep roads after my first play through lol. Nice find

Good thing I knew about the way to prevent the trigger from happening. Without saving (save for what the game autosaves) I left the dungeon, talked to the guards to resolve the quest, went to the Hanged Man (I do not know whether it was day or night though) sounded the horn, swapped out Isabella, made sure to check the abilities section of the character sheet to confirm the bonus was no longer there, went to the gallows during the day without her in my party and then saved.
So far my cheating rogue has not shown any signs of slowing down after doing a few of the kill the gangs quests. I suspect if I was not able to avoid the bug I would have noticed significant decrease in speed, right? After all, the rogue (to me it seems) is the fastest of all classes when it comes to attacking, yeah?

also, i wonder if her attacking you in fade triggers it, too...
