According to the strat guide, this item is on Kanky, the act 2 docks gangleader, but all i get from him is money, Same when i turn in the quest to the woman in the hanged man.
Is it just an insanely low drop rate (I've killed him 5 times)? or is this a misprint and the helm is else where?

It's not there. I heard it drops during Following the Qun. When you have the random encounter with the fanatics.

It drops off the Thug Leader when you go to do Following the Qun in the ambush event prior to visiting the Qunari compound.
