I think Flemeth killed them both. They did their jobs, their usefulness ran out.
Also, it sends a message to the fanboys and fangirls, resolves appearance issues, and brings a few additional choices to a reunited zero.
i think it sends a pretty confusing message. As obvious by all the forum posts on the topic, and my gut reaction when I heard Leliana's line at the end, most people interpret it as "omg! something happened to my warden and hawke that's really important! da3 must be answering what that is, and have both of them in it!"
If Bioware really was never going to bring back the warden, it would have been better for them not to have that line in there, IMHO. They'd already established with DA2 the possibility that it may be standard for every DA game to have a new protagonist. Now they've just gona and given everyone hope.
