There are a couple minor issues I have noticed in regards to some profile icons that really isn't as pressing as some of the other issues that need to be dealt with, but on the other hand I would guess that it would be the easiest to fix.
The first issue has to deal with the Xbox.com Dragon Age 2 profile Icon (Boxart stand-in usually), or should I say apparent lack of one. Under the games played tab that lists all games played the (again usually boxart) Icon next to the gamescore listed here is apparently blank (basically black outline denoting scope of picture size wtih just well blank white backdrop). I am not sure if this is just an issue with my Xbox.com profile (as I have seen it on DarkStar One and the recent Xbox Live Labs test to name a couple) or if the Dragon Age 2 icon has simply not been uploaded to the site yet.
The second issue, one which I find a bit more annoying deals with the Dragon Age 2 icon on the Xbox Dashboard.
*(What I am referring to is the small achievement size/shaped icon that appears next to a game's name on the list of slides that you view on the games played/achievements section of the menus. This icon also is added to the 100% completion slide upon 100% achievement completion in that respective game)
What I have noticed here is that the Icon that essentially comes with the game is apparently different from the one that is currently residing on Mircrosoft's servers.
Basically I did a Gamertag recovery and the former Icon (which was a Roman Numeral 2 in the same style as the D/A icon for the first Dragon Age) was replaced with a different icon (which is that of a red backdrop and I think it says Dragon Age 2 on it).
Is this an issue that can be fixed or is it something that has to be sorted out on Microsofts servers? I have noticed that sometimes a DLC will change the dashboard icon of the game, would a future DLC revert it back to this state, or could I simply delete and re-instal the Sebastian DLC to fix it?

This sounds like some temp images are being used and it looks like Microsoft are doing maintenance on the Xbox servers at the moment (I can't access some games pages). To be 100% sure I would need to see the images, would you be able to post an image for the achievement Icon?

Basically the best I can do is describe it a bit
For Origins it was basically an abbreviated version of this

That eliminates most of the letters in order to fit in the icon
Initially Dragon Age 2 was just the Roman Numeral 2 in this same font and color
However the stock image Microsoft has is more like this

However it more or less is just the blood dragon with "Dragon Age 2" written on it (Hawke and the shadows are vacant from the looks of it)
I could probably get a picture of the image that Microsoft has on its servers but not at this very moment at least

All right that looks to be a placeholder image which got downloaded onto your gamertag because you recovered your gamertag while they are doing maintenance. If it hasn't reverted back to the II icon I would use the recovery feature again as this should get it to receive the correct image. The problem is is on the Microsoft severs.
As for the cover not showing on Xbox.com are you getting a green Xbox LIVE card instead? (I just get a blank image at the moment) if you are then it is due to the image not being retrievable and so gets replaced with the generic Xbox LIVE card. (the same happens on the Game Library box when you install the game)

Yeah the Boxart on the site is now up (for the first time I have noticed)
I have not tried to recover my gamertag again however, I will see how that goes. I have noticed though that the placeholder seems to be on the gamertags on the Xbox.com forums (as that is where I found a copy) so I would be curious to see if that ever gets remedied as well.
