In my second playthrough, I installed the new patch during Act 2.  During the Family Matter quest, the Silverite-Reinforced Buckles did not show as installed even through I got the flash of  light when Varric claimed them.  I had a previous save, so I went back and did it again with the same result.  Some other upgrades DID install properly for other team members.  On my first playthrough, before the patch, the above mention item did install properly and show as installed.  You might look into this.
