1/ On my first playthrough when i summoned my dog he would follow me everywhere until he died and i would just summon him again.  But on the second play when i summon him he will never follow me?  Ive tried everything. 

2/ Sometimes in battle when i heal a charachter the health will still show he/she is damaged?


EDIT:

3/ Sometimes at the end of combat one enemy wil be on his/her feet swaying.  You are unable to attack this person? You cannot get out of battle mode until eventually he/she dies/falls down.  Sometimes this takes a few minutes?  Once it happened during a quest so i had to wait around to collect loot specific to quest.

I was playing as a rogue and I fell through the floor, I landed underneath the qunari compound, and a cutscene ensued and I was stuck in the qunari compound and could not get out.
