I just completed a playthrough as a male rogue and romanced Isabela but Isabela's outfit never changed at all.
Did I do something wrong or is it really a bug?

It's a bug, if u have the black emporium have her dring a respec pot to fix it.

Well, this explains why I didn't spot a difference between the outfits. It got a bit confusing, considering how very different Merrill's outfit was. (Honestly, I prefer Merrill's original outfit. It fits her character better.)
