At first I thought it was my 360. But now it has happened twice and made me lose a very very good weapon has annoyed me.

The first time it crashed it was on the loading screen for when I finally confronted the Arishok. This was more than annoying, as it didn't even auto-save as usual. So I was forced to redo an hours worth of gameplay. The second time was when I was heading to the Gallows for the final confrontation with Meredith. I lost a very good weapon as it didn't auto-save as usual. 

Not sure if this game has crashed on loading screen for important events.

Yeah I have come across this issue when heading into the Gallows for the final confrontation. As for the auto-save; it might be saving but the auto-saves appear to be out of order so if you select "Load Last Save" it loads an older save. if this happens again do a manual Load and compare the saves.

The only solution to stop you losing too much time though is to regularly save (I would suggest a 4 save rolling system) until this is fixed

I ended up going back to what I did in ME. Although some times I kept forgetting. lol
