It's my second playthrough and I've noticed that occasionally, Hawke and sometimes companions will use health potions without me telling them to.  There aren't any tactics set up regarding potions, and I just confirmed that this is exactly what's been happening (just checked the menu and potions are on cooldown).  So.  Any way around this?  It wouldn't be nearly as bad if this wasn't happening when we're almost at full health...

This is happening to me as well. Although my Poultices just outright disappear, and nobody has used them. I start a fight with 2 and then when I need to use them, they're just gone.
