I have had no problem with fights (High Dragon being the exception) since I started the game on Hard (for ****'s I know) and I am finishing up loose ends before I beat the game.  I ran across a few tough fights but nothing serious, but I ran into this Arcane Horror WHO HAPPENS TO BE GOD'S DIVINE WRAITH UPON HUMANITY!!!  The damn thing instantly kills Anders and Varric, then proceeds to destroy Merrill and Hawke.  I don't really understand it neither, I have a good tank setup, good Merrill dps setup, Standard-ish Anders healer setup, and Varric for lols and I am still getting my ass handed to me.

I'm guessing this was the named Arcane Horror in Wounded Coast that you fight as part of the first Arwegian Scroll? I always found the two Revenants at the same time as part of Scroll 2 (or 3?) to be harder
