I am playing through the game on the nightmare difficulty level and run into several major issues. First is after my party members fall and are revived, every single time they are hit, unless the hit is enough to kill them, they regain back all of their hit points and all of their stanima. This has got to be a bug right? Before they die compared to the creatures on nightmare difficulty they are as weak as kittens, but revived they are invincible unless a single blow takes all of their hit points. I am up to the deep roads expedition and have just fought the dragon. Revived Carver stood toe to claw with it for literaly an hour of real time before it whiddled it down enough to kill it, and every time he was hit, his hit points shot back up to full. Is this a bug or supposed to be like this? If it is supposed to be like this the best strategy in any fight would be let most of your party get killed first and then revive them. The dragon fight brings up another issue. On nightmare difficulty level the boss fights are beyond absurd. I love a very difficult fight, but like I said it took an entire hour on the clock to kill that dragon. What about the end boss fight? Will it take 5 hours? Should I go out for dinner and come back while it is going on? Are the bosses supposed to have so many hit points it takes literal hours to kill them? Another issue is characters not attacking enemies and standing around. Is this a bug? Any help on these things is appreciated.
Revived Characters instantly heal back all Hit points when hit? Major bug right?

This is a bug, how known of a bug I have no idea but a bug nonetheless. The bug as far as I can tell for me is quite close to yours but I also have another issue. Being that I roll with Anders at higher difficulties I am constantly reviving party members, however after the most recent patch after being revived the part member will go into a kind of "idle mode". This mode has them bugged between passive following and attacking, they will constantly pull out and holster their weapon between attacks, all the while attempting to follow Hawke.
The lack of damage is another part of the revival bug, I'm not exactly sure as to why it happens but I know once the bot enters "idle mode" it requires the fight to end to reset them back to normal after entering back into passive. While I have also had the health glitch that is also part of this bug I would suggest if you reach that point I would simply reload the last autosave. The Deep Roads isn't really close to hard compared to some of the more hidden stages so trust me, reseting is a lot less time consuming than attempting to kick the enemy's shins for an hour. The health glitch is just part of a much larger bug, I just try everything in my power to keep my party members alive as best I can as a result.

Yeah, this is a pretty major bug because for anyone who plays on nightmare difficulty is going to have to use the revive spell period, and if the revive spell makes your party almost invincible, it takes all of the challange away from playing on nightmare difficulty, and really all the challange away from the game. I have played many computer role playing games, all on the hardest difficulty, including DA I, and I cannot believe bioware overlooked something this big. Anybody have any further insights or thoughts?

I honestly don't think it was an oversight because I only started seeing it happen after the 1.01 patch, I think somewhere in the patch is cause behind this problem. Plus I don't consider it making the game easier in any way considering my problem includes the party member entering into the bugged passive to aggressive cycle glitch I mention above.

As a potential work around if you hit esc to bring up the menu after ressing, it seems to fix it.
