Hardcore and insanity gameplay, squad members die constantly.

What I do is send them to cover, usually seperate from one another but sometimes together, while I take point in front of them. I get a lot of attention drawn on me but I can mostly handle it. As Sentinel I have the shields and powers to control the field. Now and then I'll direct them in front while I take out enemies from behind, softening them up for my squad to kill.

I've done an Insanity playthrough, but prefer Hardcore as my main setting. Challenging enough to keep me on my toes, but not to the point where I want to pull my hair out.

Scions though, I still can't answer to. They destroy my squad on proper. Only Grunt will ever survive them.

I've always wondered if it's a Garrus thing though, in this case. I feel like he tends to be noticeably more aggressive than anyone else in combat, and doesn't take cover as much as he really should. It almost fits his persona.

As far as we know, though, squadmates change their behavior depending on what weapon they have equipped, not on which squadmate they are in the first place.  For instance, Jacob wielding a shotgun will be just as aggressive as Tali wielding a shotgun.  Jacob wielding a pistol will behave just like Mordin wielding a pistol.  This doesn't include power usage when they are set to use powers on their own, of course.

The point is that it doesn't seem Bioware scripted any squadmates to match their personalities.

A fair point.  He has special behavior scripted in.

From my experience, the listed issues happen to all my party members at one point or another.

I just chose Garrus because I take him with me frequently.

 Simply use the tactical orders feature to make sure your teammates remain in good hiding spots. It might also help to turn off the automatic feature for squad power usage.
