Hardcore and insanity gameplay, squad members die constantly

problem I've been having is that my squad dies a lot and to save on
medi-gel I end up having to complete certain spots all on my own which
is not easy and takes a very long time. I place my squad mates in good
areas for cover so they can attack from certain points, regardless they
still end up dying a lot.
Any tips on how to progress on the
harder settings without your squad mates dying constantly? Against the
collectors they die almost immediately making it very difficult to take
on them and harbinger at the same time. I can do it albeit I have to do
it many times before completing it.

Move them further back, have Shepard closest to the enemies.
You do not have to position them manually, but make sure they are farther from the enemies than you are.

You'll have to reposition them when this happens or they will die pretty quickly.

On higher difficulties you have to monitor your squadmates much more closely. I would turn off any options where squadmates cast there own powers and handle that yourself. In addition feel free to pause the game frequently so you can check where the bad guys are ... this will give you an idea of who may need your help ...
If you notice one of your team is shot up but just ducked behind cover do NOT give them a power to cast at this time unless you really need it as they'll pop out of cover to cast the power and get shot at more [and die?].

Playing on either hc or insanity requires far more tactical use of your squad and patience. This is certainly not an assault fest where you can stand and blast from the middle of the room.
As others have said, I try and keep squad mates far away and me do the wearing down of the enemies, then have the squad move in and clear up. Otherwise, if they are too close, they will just rush of into the distance....and die... really quickly.
If you can set up a triangle of the squad, depending on the battle-area, so that enemies are in the crossfire. This isn't always possible in say the Collector Ship and later end game battle but it certainly works with the other LM's etc.
Not convinced that it really matters what specialism you are - Soldier, Vanguard etc. It will be possible to complete the game on any and you can mix and match on the squad mates using their unique skills accordingly to suit the mission. It helps here if you have had at least one full playthrough so you know what to expect.
In my second run through, I've not really made use of the heavy weapons, which are of limited use anyway due to lack of ammo. As someone said here, or elsewhere, they are really just a gimmick.
The key to most of the bosses and praetorian is to run round a lot, use cover effectively (I found it was better to stand when engaging a harbinger because of those black dark matter balls he throws, this way you can avoid them). Crouching behind cover anyway is a bit iffy, you often get concussed out and then die. I would prefer to have a crouching option which was available in ME1.
Presume that this is because ME2 is also a console version.
But as said above, PATIENCE is the key and close monitoring of those teammates.

Of course, now my squadmates have to have assault rifles or sniper rifles for maximum safety. Go fig.
