I'm trying to figure out why some of the tactics I have set up don't seem to be triggering when (or as often as..) I think they should... especially a problem when the tactic in question is "use health potion".

Can anyone tell me how the tactics screen works? Well, not the screen itself but the way it chooses tactics to use.. Does it go through the entire list using any tactic that is available/applicable before starting again at the top or does it find a tactic, use it, then go back to the top immediately (thus ignoring any tactics that come after the one used)?

It's a priority system, starting again after each action.
1) If A then X
2) If B then Y
A is met performing X.
A is met again performing X.
A is not met move to next tactic.
B is met perform Y.
1) If A then X
2) If B then Y
A is met performing X.
A is met again performing X.
A is not met move to next tactic.
B is met perform Y.

So for instance, the health potions (and other "oh crap!" actions like Mind Blast for a mage under melee attack) should probably go at/near the top, then other tactics follow, decreasing in importance to gameplay the lower you go on the list?

It starts from the top each time and does the first applicable tactic.
Basically you want your most important tactics first (threat drops, potions, heals, etc) and the rest as you see fit. I tend to put cross-class combo tactics second, then regular attacks, then sustainables, and finally my focus fire tactic.
