At the end.
During my first play through, I lost Bethany in the deep roads; Isabella never came back and I RPd Aveline staying as guard captain. (so didn't use her after the conversation where I selected the 'you seem to love this job' option)
Is there any other companions that you can effectively turn against you/they'll walk away during the game due to quest choices?
Aveline was just an RP choice, so I have a feeling she'll always be there; also given her requirement for some main quest points. But what about the others? I guess I can hand over Fenris to his former master.
How few companions can you have left?

You can lose Fenris, Anders, and Sebastian at the end. So uh... everyone but Varric and Merrill?

Varric and Avaline are pretty much impossible to lose.
Anders can leave after he kills the mage girl. He will return at the end, where you can kill him.
Fenris will turn against you if you side with mages, but can be convinced to help.
Merrill can be left to the elves, or turn against you at the end.
Bethany/Carver can be lost in the deep roads, otherwise they also are removed from most of the game.
Isabela can leave with the relic, but if she does return she is pretty much friendly enough she won't turn at the end.
Anders can leave after he kills the mage girl. He will return at the end, where you can kill him.

Aside from killing Anders, he will help the mages at the Gallows if you sided with the templars and spared his life. If Merrill doesn't have high rivalry/friendship with you by the end of the game and you chose to defend the templars, I think she leaves/turns against you too. Then there's Sebastian, who leaves for good if you spare Anders.

You can send Isabela with the Qunari and what happens there...well...only Sten knows.

Ok, so Anders is possible to lose with the mage girl; which makes sense; and of course killing the punk at the end (if not playing a mage).
Leaving Merrill to the elves: Is that part way during her quest or at the point where she really screws everything up? I was thinking of a play through where I wouldn't actually help her, but still do the quests to be able to tell her BM is bad and side with the keeper all of the time.
