I'd like to do in Dragon Age what some plan to do in Mass Effect, which
is to keep the same Shepard through the series. (or in the case of
Dragon Age, the same "world" aka plot flags)
But I've read Dragon
Age, the expansion and the DLC all screw up various plot flags. And of
course if you lose data in one campaign, it will remain lost in those
that follow. So even a patch from Bioware would not restore everything
to the way you had it.
What bugs have been discovered? Has anything been said about fixing them?
I don't want to lose sleep (so to speak) over making all the right choices only to have them corrupted.

If your on the pc I believe there is a mod to pick and choose all the decisions to import into da2. You could still play through origins, awakening, and all the dlc, but then use this mod if it screws up. So that way you can still feel like you did the playthrough but not have to worry about running into any bugs.

Anything known about if also the Ultimate Edition suffers from this?

Well, the mod isn't able to fix everything unfortunately. So yes, your perfect save will have issues. It's a DA2 issue, not a DAO issue, so all versions will have problems.
Edit: Well, there are a couple of DAO issues (boons not being recognized properly if Alistair is king is one). But whether you play the expansions or not, there are issues. There may be some different ones if you do all the expansions, but even Origins imports have some problems (most notably the love interest cameos).
