So i got tot he room of death and stuckyness.  I get in, defeat the Demon and the 4-5 Lesser Shades, have done it 5x now (all on same character) with mixed results of Ser Otto dying and/or living.  after teh fight and the speech about continuing on though i can't move forward.  Have read info about takign thigns from chest, but i can't open the chest, and both doors are locked and need a key.

tips, hints or suggestions on what to do (please don't say replay the last 20hrs of gameplay) 

oops - xbox 360 version, so no fancy console codes.
