It seems the autosaves are not working correctly. For example last night the game autosaved before a large fight. I went in, fought, and failed (died). I clicked "Load Last Save" and it loaded the last save I had saved, which luckily and unluckily was the beginning of the quest (a deep roads quest where the last fight is with all those demons).
I've had this happen maybe 2 or 3 times now. It's like it loads the last save I saved, not the autosave, which is the last save. This time it was even worse, because as soon as I loaded, it autosaved, so I have to redo the entire quest.
This doesn't seem to be a problem on the PC, though I can't confirm that, but a couple others have reported this problem on Xbox360. Some suggest it's a timestamp problem, in which the game for some reason believes the manual save is after the autosave. I can confirm the autosave before the fight, which means it should have loaded that, and confirm that it did not.
I hope this helps resolving this issue. If not, I guess I'll have to just manually reload if I die.
