You can run the mod during the game. It will not then affect NPCs in the party of course, but it should affect all the others.
I altered Anomen to a single class fighter using the mod. Then I loaded a save game, there were no changes apparent. Said save game had the party at the Copper Coronet, where Anomen is found. However, after the party left for the Docks and returned, the changes were seen alright.

Wow! This is great. Tackled my BGII NPCs last night - even more fun than BGI. It's almost like being a Dungeon Master again – something I’ve not done since the early early-mid ‘80s. 
Oh, and get this!
This is cool - before I used Level 1 NPCs I added Kivan's and Sarah's Mods. So, when I added level 1 NPCs I got to mod Kivan, Deheriana and Sarah using Level 1 NPCs. I brought Kivan over from BGI as an Archer (same I had him in BGI) and added Deheriana as a Druid - you know, because Druids and Rangers are Outdoorsy types so that's how I say they met and fell in love. Also, looked at Sarah but kept her as a Ranger - just moved a couple of stats around to balance her out a bit.
I also:
Dualed my BGI Swashbuckler Imoen into a Mage/Thief (Swashbuckler) for BGII. Seems to fit the storyline best.
I've given Minsc a Barbarian Kit for BGI and BGII. I always thought Rashiman sounded like a place where Barbarians come from.
Repackaged Yoshimo as a Kensai. So cool.
Left Anomen the same but balanced some of his stats w/o changing his total stat points.
Made Valygar a Wizard Slayer to fit his storyline better.
Left Nalia alone but looked at her closely. Not sure how changing her would affect her opening comments to the PC (I realize the mod accounts for changes in NPC kits). I kinda like her as a thief/rogue but fear I need her spellcasting (until Imoen gets back).
Left Mazzy and Aerie alone too. They seem about right. Don’t normally use them anyhow but like them for a future (mostly) LG party.
Wow, it just occurred to me I made Yoshi a Kensai, i.e. no more thieving skills. Now, I've gotta go back and look at Nalia again.
Thanks to SoI, TheHun, Grond0, Ishad Nha and especially ZenMonkey for starting the thread that led me to find this mod. So much fun.
Boy, if this mod allowed you to change alignment the possibilities would be endless (I’m a LG kinda guy).

The trouble I have with the unmodded Nalia is that her thief skills are not good enough to be much use without potions. Hence she tends to be used only as a pure mage. I use the mod to make her into a pure thief. She still duals to mage early in the game, but a few more levels gives her the opportunity to find her niche as a thief, e.g. in open locks / find traps or as a specialist in detect illusion.Charlestonian Knight Templar wrote...

I modded Nalia too for the same reasons. A pure Thief like Yoshimo does not grab me, a Monk can't open all those locked chests that you are always finding. Her first class enables her to use Thief weapons and gives her a few more hp. As a Mage she will be wearing Bracers of AC#, they won't interfere with her thieving skills.

Yup, gotta go mod Nalia to a Thief tonight. Then I can finally start playing and stop modding.
