Can anyone tell me where to get chapter 3? I see it in the chantry during the fight after the vicounts son is killed but I can't get to it because the fight happens right after you see it and disappears after fight. Is there another time when it can be acquired?

You have to read it during that quest, during the fight or (if you are quick 
) in the brief moments between the fight end and the cutscene. You won't be able to click it during that narrow window unless you end the fight near the altar though!

I have yet to try it but they say you can snag it during the fight by using the radial menu---you can "manually target" the book that way even during combat.
