It's promotional DLC, namely the Umbra Visor.

For more details, have a look at the mass effect wiki page.

Probably not, I think it was a USA offer, and I don't think they are still offering them now, because it was last year that I got mine, around summer IIRC.

you can buy these on ebay. I won't post a link because I don't know if it's allowed but just search for dr pepper dlc and you find some offers. I bought mine a while back on ebay USA as well (german account works there as well and you can usually pay with Paypal). You can get all 3 codes for 1-2€ but redeeming them is a bit complicated. You need to create a Dr. Pepper account first (and as far as I can remember I needed to create 3 - one for each item, they are free though) to redeem the code and then be redirected to the Bioware page to link it to your account. Just ask for a description how to redeem them excatly when buying them. Before buying check for which version it is, usually it should be for Xbox/PC (they work for both then). Don't know about PS3 versions, I think they are already included in the PS3 version or are being sold as separate DLC in the PSN.

USA only unless you can get someone to buy some dr. pepper and give you the codes.

And it is still going on. Check the link. It restarted with the release of the PS3 version of ME2.

It's available as purchasable DLC on the PSN if you have the PS3 version.

Are the Dr. Pepper codes in every Dr. Pepper bottle you buy? Or is it a gamble, where you must keep buying bottles to have a chance of getting the code?

here are the codes and directionsfor all 4 free items.
The codes are as follows:

CBEEAAA41NWH
CBEEAAAE4L9N
CBEEAAAFQG9J
CBEEAAAE98ZV

To apply these codes to your EA account and redeem the Mass Effect 2 DLC items, simply create an account on Dr Pepper’s Promotion website, fill in whatever fake details you please and simply use an @mailinator.com address for the e-mail field. The site will never ask you to confirm your e-mail address. Once you’ve logged in, simply enter the promotional code and pick “No Purchase” for each of the three fields.

When you’re done, just hit the button to redeem the DLC and log in with your real EA account details, select Mass Effect 2 from the list of games, and finalize your selection.

To redeem the remaining two items, simply repeat the process with a different code and e-mail address for your new DrPepper account, always logging back into your real EA account at the end to claim your item

Those codes are old codes, doubt they'll work anymore.

Every bottle of a certain type.  Not all the Dr. Pepper bottles will work.  It needs to have an EA symbol on the bottle and say something to the effect of every bottle wins.  It will generally have a yellow cap (at least around here)
