they were all loyal the last 2 times i finished the game but first time tali died and second time mordin

Who you pick to lead teams, as well as who you pick to do certain tasks makes a difference. Picking a non-leader type character to lead the fire team might end with a character dying, or having a biotic that isn't strong enough to hold the barrier around you during the seeker swarms may get a character that comes with you killed.

well the barrier never been a problem samara or jack took good care of that...

Characters with leader experience are better for leading the fire teams than other characters. Examples being from my personal experience.
Tali or Legion are the best hackers for the tunnel.
Garrus is whom I use to lead the fire team. He's had experience leading his own team, as he tells you. I believe Zaeed is also a leader character.
I haven't had anyone die on the escort back to the normandy.
The Barrier is done best by Samara or Jack, Jacob isn't powerful enough to make it.
The rest is basically free reign.There are guides on youtube to surviving the suicide mission, that will explain in more detail.

He is not. If you take the paragon route in his mission he basically tells you that. Although I didn't think about it on my first playthrough and picked Zaeed for the second fire team. He ended up dead. Miranda and Jacob along with Garrus are ideal for leading the fire teams.
Also to note, there is a small chance Mordin will die holding the line even if he is loyal. I'm not sure if it's possible if you have all squad members loyal and don't lose anyone, but the safest thing is to either choose him to escort the normandy crew or take him with you to the final fight.

all my playthroughs i've done, i've NEVER had mordin bite the big one, even at the "hold the line" portion of the SM. but, i usually have all squaddies loyal before starting the SM anyways.
anywho, @ OP. there are many guides out there to get everyone to survive. assuming you have all the normandy upgrades, and all squaddies loyal, the setup i run with on all my playthroughs is as follows:
tali for tech expert in the tunnels
garrus leading the fire team the first time.
samara and legion as my squaddies
jacob on the escort back to the normandy
jack as the biotic for holding the shield
garrus as fire team lead (again)
samara and legion as squad (again)
using the following in all my insanity playthroughs, i've never had anyone die. sure there are other variations you can do, but this is the one that i like best, and thus suits me.

Before I ever looked at the forums or guides, I sent zaeed to escort the crew back to the ship and kept choosing Garrus as the team leader. 3 play throughs and mordin didn't suffer a scratch once. Not when I left him behind to hold the line, nor when I took him to face the terminator.

First task: hacking
Kasumi, legion, tali
Fire team leaders: miranda jacob and garrus will always live
Have mordin escort the surviving crew back to the normandy and he will always live
Make sure leave at least 2 of these character behind to hold the line: Garrus, Grunt, Zaeed
And use jack or samara for the barrier

People often overlook bringing Mordin to fight the Human Reaper instead of sending him as an escort or holding the line. His incinerate power makes him the most useful squadmate, along with Miranda's warp. And you get rid of two people with weak hold-the-line tendencies. I normally let Tali escort the crew, everyone survives.
Also, does anyone know if the damage protection, Krogan vitality, geth shield strength, etc. upgrades affect hold the line survivability at all?
