It happens to me sometimes. I don't know the exact conditions, but the last time, Hawke has fallen then Anders use the revive spell. Afterwards, pushing R to attack nearest mob with Hawke has no effect, I have to click to attack. What's annoying is, it does only one hit, I have to click to attack like an hack'n'slash lol. It's funny to play DA2 like Diablo but pretty hard, specially on nightmare
I found a way around it. You need to change the character's tactic behavior (tank, agressive whatever) and play as someone else. Now the character will be played by the AI and when you control it again, auto attack is back.
