I romanced both Isabella and Merrill but i dumped Izzy after sleeping with Merrill.  I thought Izzy would go back to just being friends but in Varric's final words when he says "All except for.....", i can clearly hear him saying both Merrill and Isabella at the same time.

Is this a bug or does the game just not remember me breaking up with Izzy?

A known bug. The one you first sleep with will usually be mentioned by Varric even if you end up with someone else.

Possibly, but since Isabela is usually the first one most people sleep with it's only been reported with her.

Isabella was first but I had relationship with Merrill and Isabella is mentioned only in the end, she left me ages ago after taking that artifact so this is kinda sucks.

i think it's an isebella bug. i havent seen people who dumped fenris and go after anders report this.

That actually makes sense. Think about it; Merril is innocent, trusting, naive and gentle. Everything Isabela isn't! They're yin and yang!

And yes, I got that bug too. Looks like the Isabela flag isn't resetting once you break it off with her.
