Quest is started by reading a letter on his desk in act 3, then questioning him about it.

I missed that quest first time around, too.
You have to click on a letter in Gamlen's house . . .not sure which act.
If I ever resume my second playthrough, I'll be looking for it.

Yeah, you can find her in Act III. There's a note on his table that starts the quest.

Wait a minute, Hawke has a cousin? When do you find this out? I completed most of the quests and popped in on Gamlen every now and then but never heard this!

yup. there's a letter on Gamlen's table that you can read in Act 3. He says "Mind your own business!" or something, you talk to him, ask him about the item in the note, pick the smartass option (I do anyway), and boom. Quest to find long lost cousin.
The item is, in all honesty, worthless. Don't pick it as a reward. It'll just become obsolete later on anyway
I love being a smartass. Especially in this quest as the culmination begins to happen.
