At the moment, I can only recall Gascard, Pertrice and Grace getting under my skin...Grace especially.
My Hawke was able to lie to Ser Karas and give Grace and co. a good opportunity to escape with minmal bloodshed. She proceeds to get captured anyway, and then says that it's my fault for giving her such little time to escape! Then, to top it all off, she takes part in the kidnapping of Bethany! That's just - I just....ARGH!

1-5. Petrice! I really hate this...I should have killed her, not the qunari.

There are a lot of characters to hate in DA2. But I'd say the Arishok is the douchiest of them all. He had no intention on leaving ever and wanted to force his culture on a foreign country. And no there is no freedom in his stupid qun, it's either submit to the qun or die.

I really don't understand the love for the Arishok. All he has going for him is a cool voice. Other than that, I find him at best a dangerous, but somewhat honorable zealot. The codex entry that spoke of what happened to Kirkwall during the first Quinari invasion made it sound absolutely horrific, with all of the surviving people left "hollow eyed, stripped of free will." Although I hate Petrice's smug self-rightousness, I agree with her on the point that the Qunari should be removed, by any means, as quickly as possible. They say they give people a choice to follow the Qun, however, if they actually invaded, the only choice would be "Follow the Qun" or "Become a Mindless laborer." I find the stripping of free-will, and indoctrination frankly horrific. To be short, he's on my list of hated characters because of what he stands for. Also, as he is part of Kirkwall, he has a responsibility to obey the laws of Kirkwall. Simple as that.

Jeez, almost everyone in the game is on these lists. Methinks that the lead designer got all the character writers in a room and said to them: "we want these characters to be complex. I want you to write down 2 or 3 contradicting characterists for each NPC and build that into your writing. These kids will be so confused it will be delicious!"
A little too 'thinly veiled complexity' for my tastes. I would have rather seen more nuance than controversy in these characters. You know, sometimes the characters should do or say something which makes you think but you can't quite put your finger on it, or makes you want to reload and hear what they had to say again. It's like the game beats you over the head with a hammer to point out how controversial every character is. Working as intended?
As for the list, I'll list a coulple notable gripes...
Arishok - Dumber than a bag of hammers. 7+ years sitting in a slum waiting for some relic to appear not telling anyone what it is they're looking for and seemingly doing nothing in an effort to find this relic. What were they waiting for? A basket at their front door? I believe he was written to demonstrate intellect and culture-centric wisdom, but his endless circuitous logic ruined the effect for me.
Grand Cleric Elthina - Useless. She may be a spiritual beacon of goodness, but she is no leader by any definition and shouldn't head an organization if she can't lead. Reminds me of a woman I know at work... she is always in la-la land so pleasant and oblivious it just makes you want to go up to her and slap both her cheeks at the same time and tell her to wake up. We have work to do goddammit!
I write up these two because I think that I'm not particularly impressed by how they were executed (no I don't mean how they were killed). Most of the other 'hatable' characters are working as intended in terms of their impact and character. But like I said earlier, the complexity is a bit too obvious and predictable.

10. Danarius. Dude, just let it go already. It's been more than 6 bloody years.
9. Grace - I sympathized with her early on, but god DAMN, she can hold a grudge. Also, she ruined Thrask's goals and killed the one man who might have been able to defuse the whole stupid crisis, if he'd only had more time.
8.The Arishok. He was badass, sure. But he was also stupid, arrogant, cruel and generally impossible to work with. Also, he was a **** to duel.
7. Seneschal Bran. God, what an ****.
6. Kelder. Mental illness buys him a little sympathy, but he was still a major sicko, and the world is well-rid of him.
5.Huon. Creepy motherf**ker. Cutting him down felt awesome.
4. Petrice. Noble intentions or not, she was a scheming and manipulative ****, and she honestly wasn't even very good at her job.
3. Knight Commander Meredith. Mental illness keeps her from being number one, but she was still a paranoid and schizophrenic tyrant.
2. Quentin. I hate crazy mages as a rule, but he was just monstrous. His death was too quick and painless by far.
1.Anders, Anders, ANDERS. The murder knife was too good for him....I wanted to cut him into a thousand pieces, slowly. He's the most stupid, annoying and criminally hypocritical character I've ever encountered.
(Orsino gets an honorable mention because of that crazy-ass Harvester **** at the end, as well as his secret correspondence with Quentin. Also Tarahne, for being waay too creepy)

1. Sebastian (I wanted the murder-knife option at the end. Preachy prig doesn't get to threaten my LI)
2. Petrice (I have so much hate for her, but oddly, I enjoyed her VA)
3. Aveline (seriously, ALL her quests were ridiculously annoying, and she b*tches constantly. She would have been a better character if she were a man. Too bad Ser Wesley died instead of her.)
4. Quentin (sick-o freak cut up my mom!)
5. Elthina (Hope you found a balm/bomb for it. Yep! Sure did.)
6. Alrik (for obvious sadism)
7. Huon (I burned my eyeballs looking at him)
8. Fenris (you HATE mages, I get it already!)
9. Danarius (Look what you turned Fenris into, you bastard!)
10. Meredith (she's just pure psycho)
There were many more, but those were my tops!
EDIT: I forgot Grace. I HATED that witch, and I hated her plot armor. GRRRR.

I'm surprised Gamlen isn't higher on the hated list. :/
I'm also surprised that Carver is so high on the list.
Yeah, Carver is an ass, but he's your brother, and he plays a good brother, if you get my meaning. If me and my brother were in DAverse, I'm sure me and him would be at each others' throats like, say a mage Hawke and Carver.
So... where do Meredith and Orsino rank? I didn't like either of them; or rather I hated Meredith at first, but then she redeemed herself a teeny bit. I liked Orsino at first, but then he went and threw that back in my face.

Sebastian. I don't hate him because he preachy, I hate him because he is an absolutely coward. He couldn't be bothered to deal with his family's killers himself, so he hides in the chantry, perfectly content to let Flint Company go unpunished. He didn' t have the balls to turn in Anders and Merrill for fear of falling out with Hawke, so the douche tries to convince Fenris to do it. Since Fenris turns him down, he leaves the matter alone. Finally, instead of dealing with Anders himself he swears vengence and vows to return with an army he probably won't even be able to raise and hunt down one man.
I'm sad I didn't have to option to laugh in his face when he threatened to return with an army.

After finishing the game... I'd like to put Merrill on this list for the last part of her questlines "Why didn't they let me help them?" Gods Damnit Merrill you still haven't been listening!? YOU ARE NOT ALWAYS RIGHT. IN FACT YOU WERE VERY WRONG AND PUT EVERYONE IN DANGER BECAUSE YOU WOULDN'T LISTEN TO REASON
GAH
probably not helped by not being able to respond to that in a way I would have liked... best responce I could see is 'you can't help people without their consent' I didn't want to console her, I wanted to chew her out. Because she still hasn't learned!

In no particular order:
Grace: Ungrateful b*tch (at least, in my first run where I did a lot to cover for them)
Petrice: Yes, lets incite a religious war with the heathens... short sighted twit.
Fenris: Belligerent ****hole.
Gamlen: Starts off okay, but turns into a vindictive jerk.
Meredith: Not for her psycho sword thing (which honestly feels like an "out of left field" moment) but for her immediate hostility towards Hawke.
Orsino: I'LL SHOW THEM BLOOD MAGIC! nuff said.
Carver: The "in the sibling's shadow" act is really annoying.
Most of the other characters that I don't like aren't on screen enough for me to build anything more than a superficial hatred of them.
Edit: I forgot to mention Senechal Bran. He's a complete d*ck, too.

I hated the fact that, throughout the game, the suggestion of rape was around almost every damn corner. But I suppose that's not what this thread is asking, so...
My dislike for companions mostly depended on my different approaches to playthroughs. It really only mattered when I was supporting the mages as a mage. I greatly disliked Fenris and his anti-mage rants, along with Sebastian's pro-Chantry preachings. Carver was awesome, in my opinion. I relished in having the opportunity to actually argue with someone and get my way. Sort of.
However, in every playthrough I somewhat disliked each of these characters: Isabela, whom I felt I needed to cater to in order to win her complete friendship or rivalry; Merrill, whose bullheadedness just made me want to facepalm; and (J)Anders, who I found generally annoying and disappointing, especially when compared to Awakening's Anders.
NPC-wise, I agree with the others who hated Petrice's plot armor, because that was absolutely infuriating. Bartrand was an arse inclined to extreme depravity after having found that idol. I liked Gamlen, mostly because he was just an approximately 55-year old Carver.
I think the only character I haven't felt any negativity towards is Varric.

Yeah. <_< Urgh. Nobody in the game gets on my nerves as much as Grace does. Possibly, Fenris might be another equally annoying character IMO, but his overall nice character design, if you subtract his personality, sort of balances that out.
Grace killed my favorite character, and the only position that I support.
DA2:
1. Grace
2. Sister Petrice
My DA:O "Most Hated Characters List" looked like this:
1. Arlessa Isolde
2. Anora

i'm not sure if i really can point out to many, fenris is the only one i can think of. but dragging him and anders around the city is pure hilarity.
fenris: i remember you saying something earlier.
anders: shut up.
fenris: "i can control it"
it would be easier for me to list favorites.
why is anders on the list? i like him so much more in this game rather then awakening, before he couldn't take a single thing seriously, and when i talked with him i would end up pissing him off cause i couldn't tell when he was joking or not. my two cents of course.
