Hello all,
I have tried my best to figure out what tactics to use to deal with Flemeth when she grabs me or one of my party members and starts swinging them around and chewing on them. Every time she does this me or one of my party members dies. I think I could kill her if I could just deal with the grabbing and chewing she does on me and my party members. Do you folks have any advice on tactics to use to deal with this?

The only thing I can think of to do now is to use the Revival abilty used by Wynne. How do I set up the tactics to use it to revive my fallen party members?

Both Dog and Shale are immune to being grabbed, by Flemeth or any other dragon/'grabby' monsters. Use one of them or both to keep her busy in melee, while your other party members fight from range. Spread your ranged party members out she doesn't damage everyone with her fire spit. Unless you're a healer mage, Wynne is pretty essential. Fire defence crystals with ice attacking crystals for Shale and Greater Warmth Balms are absolute life savers (prioritise melee characters if you only have 1 or 2). If you've already taken down the High Dragon, Wade can knock you up some fire-resistant armour if you're a melee charctacer. Not sure if you can set Wynne to Revive automatically, but she can only cast it once in a blue moon anyway, so it's not much of a pain to order her to do it manually. But don't let her waste her mana on offensive spells - she'll need all she can get just for healing, unless you have a ton of spare lyrium potions. Also (and this is a general rule) try to make sure you heal when party members' health has only dropped a little (say health around 70% - ish). It gives you more time to recharge heal spells and cast them again than if you wait until party members' health is low before healing.
With that, and a bit of perseverance and luck, you should be all set.

First, don't despair...it can be done!
A lot of good advice has already been posted here...remember that Dragons in DA don't like cold and are immune to Fire damage.
She can be flanked, so try to position at least one hard-core Fighter beside her but even they will be knocked back a couple of times.

I've only done one playthrough where I tried to kill her (I'm doing my second playthrough now and I just made the deal and lied to Morrigan) and my Party make-up was Sten, my Fighter character, Wynne, and Morrigan...the two Mages had Cold spells and one had that Mage's Staff that is all cold damage (I honestly can't remember the name of that Staff right now)

Thanks all for the good advice...much appreciated. It's very frustrating when she grabs me or another of my party members and does what she does, but it's still a fun and challenging fight. I don't have Shale yet, but I do have my faithful hound. I can probably bring along Oghren too, and Wynne, and use those cold spells. One good thing about DAO is you don't have to do things in order. If I still can't seem to win the fight right now I can wait till I have Shale to bring along.

Whew! Well, the dragon(Flemeth) is finally dead.
I think what really made the difference was bringing along my war hound Hercules and using a lot of health poultrices. He kept Flemeth busy some or most of the time while I tried to get in some hits on her flanks and trying to avoid her head so she would not grab me. Oghren fell unconsious. Wynne was also still alive. I kept her at range using heal, group heal, mass rejuvinate, arcane bolts, and stonefist abilities. I also enjoyed the final cut scene......nice touch.

I did this easily with Alistair, Wynne, and Shale in my party. I was a dual weilding warrior. I pretty much played as Wynne the entire fight and constantly healed party members. Alistair and Shale both died during the fight and then Wynne died while I was trying to revive them. My warrior then relied on health poultaces and finished Flemeth off by spamming abilities. It was a vary hard fight during my first attempt as a level 13 shield and sword warrior, it was much easier while playing as a 15 mage and a level 16 Dual Weilding warrior.

I have been trying to do this quest as well recently, but the grimoire seems to have disappeared from the tower. It is supposed to be in the chest in the First Enchanter's office, yeah?

If you really want to make it a boring fight:
- give your tank fire resist items
- place him just outside of Flemeth's melee range (she can't move anyway - if you hadn't noticed)
- keep taunting and maybe let the tank engange in ranged attacks, but dont move him.
- Flemeth will focus on your tank and since she cannot move, she will be using ranged firespit etc which really doesnt do much damage
- maybe Force Field the tank when he is being pulled towards Flemeth and is about to be eaten
- everyone else can test their newest shiny bows of their liking
But since there are only 2 real dragons in the game, I suggest to only do this when you just really can't beat her.

I used a tactic where alistar, was bashing Flemeth from behind and my tanker was getting chewed by her from the front, had to heal the tanker like every split second by pausing. Lelian i placed near the river and she was completely untouched and used ice arrows. Wynne was bombarded by fireballs, so healing her is a "split second" priority too. Try to keep her buffed with all the fire resistant balms. You'll need alot of mana! Took me atlist 10 trials to get the dragon down.

Cool, i used Wynne and Leliana as ranged and Alistair as Tank and meownself as dual wielding agile Rogue running around her and pinprick and kept her attention, i died once but as i have Wynne swimming in Lyriom potions she could revive me to continue harassing her.. light as a feather sting like bee eh. odd enough we took her down on the first try. All singed and smoking but alive. And lovely Morrigan couldn't keep her hands offa me when we got back to camp. Very nice young lady that one... heehee. I had a D**n lot of more trouble with Arl Howes men at the warehouse in Denerim though. took me 10-12 tries before i got the hang of it. But isn't that the beauty of the the game eh..?

You need 3 ranged regardless due to tail swipes and other AoE melee abilitys making melee classes pretty much nothing more than a liability (but you need a tank obviously).
My tactics used:
Spread the 3 DPS / healer out near the hut, making sure all of them can see Flemeth to range attack.
Get your tank to run in and hit her a couple of times to draw threat, control your tank and move him to the far left of the platform so she faces him, let him hit her, move him to the far right, hit her, back to left, etc This stops her pulling one of your ranged DPS into the fight, stops your tank taking flame breath / balls and reduces the melee attacks she does due to the jumping animation.
When she grabs your tank and nom-noms his face, i throw a barrier up. Makes her drop the tank and immune to damage for a short while.

Cone of cold freezes her for a few seconds, and idk if petrify works.

I found the key to dragons is greater walm balm as it pretty much negates the fire element of the damage dragons do. This significantly reduces the overall DPS the dragon dishes out hence your healer can more easily keep up with the healing physical damage they deal to your fighters. I went with:
Me (mage/spirit healer)
Lelliana (shooting ice arrows)
Alistair
Sten (wielding Yusaris)
Sten got chewed up once when I was caught short for a heal or mass heal but I revived him and he ended up delivering the killing blow.
