Visit Gamlen's house in Act 1, and click on Dog. You'll get the option after a small amount of banter.

I named the dog Lady.... but everyone kept calling the dog "boy" so it was apparently male? Hmmm.... Well, everyone goes both ways anyway in this game ;D
