I have used the EA download manager and completed downloading the game. How do I proceed with installing?

Once the game is downloaded, the EADM will then display an "install" button, IIRC. You may be able to learn more at the EADM forums:

The EADM after downloading the game didn't display the install button, but the play button instead. So I had to install the game manually (unziped the downloaded folder, found the install exe and installed the game). It never worked properly. When loading it in fullscreen mode it showes nothing but black screen (sound is ok). I have to load it in a window and then go fullscreen from the option in the game menu, but right after the trainer it crashes.
