Playing through for my first time and all I have left to do is to recruit the help from the mages. I proceed with the quest and encounter the sloth demon and enter the fade. A little while in it freezes and I have to start the whole quest all over again. Even when I saved right before encountering the sloth demon and entering the fade! I play through it again and I died fighting a couple of abominations. I say oh well I recently saved, So I go to reload my last save and its gone. So, again I have to restart the quest!! So, my question is, do I have to beat the whole quest without dying, since just recently saves are dissappearing. Also, when I finally entered the fade again the game froze on me again and I had to restart the whole entire quest again WTF!! Please help I would like to finish this epic game and thank you. Game is clean and its installed on my hard drive I don't know why the Fade part keeps freezing on me.
PS: Could this be because I just recently downloaded the update or has this problem been reported before?
Can't proceed in game/bug?

I would say you have a couple of problems, and it would depend which you are trying to resolve: the game freezing, the disappearing saves, or completing the Broken Circle quest.
Game freezing: typically I just switch off console and reload. Not a great deal can be done as far as I am concerned. (I would also suspect particular models of the 360 are more susceptible to these problems than others.)
Game saves: may I ask you how you continue after you die? BioWare games seem to work in a peculiar way (i.e. wrongly) when you choose to 'resume' as it often loads the wrong save file. This may give the illusion of your save disappearing. In such circumstances a manual load (which I do now) is preferable to the game's own choice. If you do manually load then something odd is happening.
I would also suggest saving frequently if you do not currently do so.
Completing the quest: you can die and reload to complete the quest. But...in a sense you do have to complete it without dying, if you see what I mean. (Meta versus game point of view as it were.)
