You have touched on a reason why I'm not a fan of GRRM. Too much killing to make "plot"? I think it's a little cheap. That's why I actually loved what happened to Bethany in my playthrough - sure, she *could* die. Or she could get stuck in the Circle, or with the Wardens, which are good plot points without killing.
That aside, it just wouldn't work. If you devoted that much time to it that it became part of the plot, you have a few catch points that would cause it to be a mess because it wasn't always going to happen. Just imagine THAT mess. As is, people complain about being railroaded into choices. I can see it now. "What?!? I was female and romanced Isabella! What happened?" Maybe one of them is secretly an Asari!?!
Then you get to the other side of it, where a male Hawke with a male romance are massively lacking parts.

I can see it now, male Hawke and Anders or Fenris looking at each other in horror. "If that's even possible in some crazy way... 3-2-1-not it!"
In either situation, a pregnancy/child would force someone into something that doesn't go with the choices they made. And sure, while those things do already happen in games, I don't think it's one that would be needed.
And the Grey Warden point is fabulous.
