Any fight where I wasn't paying much attention because I'd been playing for the past 4 hours and got killed.  Honestly, the only fights I had trouble with really came down to me not paying attention to party health.

Hybris was the most difficult in terms of sustained tactical play, I thought. The Arishok might have took longer, but that was just tedious, run run run run disable attack attack, run run run run etc.

A well-equiped Fenris in the Fade when you're playing a mage. Damn elf jump-hits me once and I go down, unless I have perfect timing and pause/cone of cold to slow him down.

Made the mistake my first time through in doing that room last, having already lost Anders and Isabela to the other demons. Never have I gotten a critical failure message so quickly into a fight.

Hardest? Arishok with a warrior. I kited around a pillar, used combustion grenade and then went into my bursts and repeated that process. It took me 15 minutes. the arishok kept healing.

I found the High Dragon ridiculously easy on hard/nightmare. Just spank and tank, use weapons with an ice elemental weapons, stay under the rock that the dragon sits on when it starts shooting out fireballs... move all your other party members (who should be varric, merrill and anders tbh) on the ramp while you get rid of the dragonlings/dragons it summons. rinse and repeat. I swear, on my last playthrough I killed the high dragon in about a minute in a half with the bloom great axe with some runes on it.

the rock wraith is definitely more difficult than the high dragon imo.

Duel against the Arishok with a mage. I ended up "cheating" by calling my Mabari...

The Arishok fight was pretty tough the first time I tried it, but subsequently, using better tactics, it's not that difficult.

A good idea is to move your entire party to the right side corner of the level where you are when the battle begins - so that the enemy will have to run down the stairs to reach you. Get them nice and bunched up that way, and some will stay behind on the top platform.

Then target the Sarebaas first. It's also helpful if you've made sure that your party has high electricity resistance prior to the battle.

Read: "duel", meaning one on one combat. Hence I said "cheating" because having my dog magically appear out of thin air to help Hawke is what I consider unfair advantage.

If it was group combat then the Arishok would stand no chance.

as a mage,
I second Xebeneck.
I had no idea what was coming my way.

Also, The High Dragon was near impossible for me, without sufficient healing and fire resistance.
It destroyed me, Merrill, Varrick and Anders the first time; Then i went and splurged on fire resistance and like 50 restoration potions and it was easy.

I'd say Meredith. It wasn't hard, it just took a really long time despite having eight of my companions, Cullen and the best equipment in game.

Xebeneck. There are too many waves of enemies.

For me it was the High Dragon at the Bone Pit mine. It always takes me several tries and sometimes new party members to finally defeat that one. Where as I made it through the final battle in one shot.

Im a pro so I dont find any of the fights hard.. even on Hard /flex lol. Havn't tried nightmare yet. But damn that Meredith fight went on for ages. It was extremely difficult for me since I had this bug where she stuns all my companions to talk.. but they would remain stunned so I couldn't do anything accept with one of them.. So I hard Merrill running around trying to avoid Meredith and cowering from her.

Arishok fight was hard the first time round but easy once I got the hang of it, but long once again.

Orsino the harvester. He just LOVED using the reaper's cudgel. Therefore, I stabbed him. Repeatedly. With a very big sword.

Will have to admit that one of the main fights for which I found was the hardest was the pride demon that you come across when you look for and destroy all them books. I had to reload and change tactics a few times with that one there. As for the High Dragon in the bone pits that was a great fight but not to much of a problem as I had Hawke<warrior>, Issabela, Feneris and Anders in the party. Everyone had weapons for which dealt cold damage and Anders has that ability for which gives all party members the elemental damage from whatever staff he's using. Also everyone having a rune of fire protection helped out allot as well.

THIS.  I bested ser Varnell on my first try, died during the high dragon battle just once and during this fight I had to reload like 3 or 4 times.

Hands down the Rock Wraith. I've spent the last 40 minutes figuring out ways to prevent myself from toning down the difficulty during that fight because for the first 5 attempts, he'd wiped the floor with me in a matter of minutes. Fortunately, I was able to persevere, but just barely. It's difficult to try moving ranged characters away from him. He was always targeting my mages/rogues rather than my tank, which is absurd because he's (tank) been on Taunt since the start of the battle. The wraith also inflicts insane amount of damage. When he does that rolling move, he'd stun/stagger a character for about 10 secs, then proceeds to shatter earth and mercilessly rams them down to death.

Rock Wraith and Arishok. The Arishok is waaaaayy harder then the supposed main antagonist...

High dragon is easy. Completed first time, but reloaded and did it on easy because it's corpse was glitched and I couldn't get my champion chestpiece :(

Rock Wraith is brutal. Always takes me at least 2 tries.

Act 1: Tal-Vashoth leader (easy if you kill the Saarebas quickly). Rock Wraith has lots of HP, but not much else, reminded me of the Hell Wyrm "super" boss of Final Fantasy XII.
Act 2: The Gauntlet (Nexus Golem's trial), and the first spider encounter in the Varteral's cave (****ing Wasp Spiders!)
Act 3: Xebeneck. So many waves with lots of Elites...

Xebenkeck for me. I'm actually surprised to see more people didn't mention it. Particularly if you're like me and stumbled into it fairly early during Act 2 without the optimal party configuration. It pretty much went like this:

Desire demon. Ok, no problem. And 2 rage demons? Whatever, I've got frost spells and so does Anders. Killed one rage demon. Wait? Now there are 3 of them? And a revenant? And an abomination is chewing my foot!

*sigh* I think it's the constant waves of Elites that do it for me.

In DA:O the only duel that was actually hard was Flemmeth...can't remember how many times I had to reload...

DA2 on the other hand leaving a bit less tactical than the other has actually more hard fights on my point of view.

The second to last fight. I don't remember what the demon thing was called but it was on my way to battle Merideth and It was the last battle before her. I blinked and my whole party was dead >:( I had to run around downing potions and praying that I wouldn't get caught in that stupid energy thing that stops you in your tracks. I was totally not expecting a fight like that.

You're talking about the Pride Demon. There's another one earlier on if you do one of Merrill's quests, so I already knew about his force cage attack and managed to avoid it. It was a pretty easy fight for me partly because I had done it already and partly because I was playing an awesome rogue who could do massive amounts of damage.

I'm not sure whether the hardest fight for me was the ARW or the high dragon. I was stuck with the resources I had with the ARW, whereas I could go home and get potions for the high dragon fight after trying it and failing. I'd probably say the high dragon was harder overall because the ARW was pretty easy after I figured out good tactics, while the high dragon was tough and took a lot of potions even after I figured it out. And Anders just wouldn't stay conscious through that fight. :P

YES! THIS PRIDE DEMON CRAP!

I didn't have healer and STUPID AI controlled characters kept walking to that damned beam of light!

Even the High Dragon was easier.

Xebenceck, and then, oddly, probably the blood mage/pride demon combo on the docks in the Last Straw.

The duel with the Arishok. Spent most of that fight running away from him while the cooling time for my potions passed. I've only played a Rogue so far, so I'm not sure if it has to do with the class. I tend to think not.

The rest of the fights were quite easy.
